What is an Advance Medical Directive (AMD)?
An Advance Medical Directive (AMD) is a legal document in Singapore that lets you state in advance that you do not want extraordinary life-sustaining treatment to be used to prolong your life if you become terminally ill and unconscious, and where death is inevitable.
It is completely voluntary and only takes effect if two doctors (including a specialist) certify your condition as terminal and irreversible.
The AMD helps ensure your wishes are respected and reduces the burden of difficult decisions on your loved ones.

Download it from MOH’s website or collect it at polyclinics or hospitals-
Make an appointment with a doctor
A doctor must witness your signing of the AMD formThe doctor will ensure you are making the directive voluntarily, without pressure, and fully understand what it means.
Submit the AMD form
Send the completed form by post to the Registrar of Advance Medical Directives at the Ministry of Health.
You will receive an official acknowledgement when it is registered.
How much does it cost?
The Ministry of Health does not charge for processing and registering an AMD.
However, the consultation fee with the doctor for witnessing your AMD will apply.
Fees vary depending on the clinic.
