What is a Lasting Power of Attorney (LPA)?
An LPA allows you (the Donor) to appoint trusted persons (Donees) to make decisions on your behalf if you lose mental capacity.

How do I apply for an LPA?
- Step 1: Prepare your LPA Form 1 online via the Office of the Public Guardian Online (OPGO). You can appoint up to 2 Donees and 1 Replacement Donee.
- Step 2: Donees must accept their appointments via OPGO using Singpass.
- Step 3: Visit an LPA Certificate Issuer (CI) to certify your LPA. Ensure your chosen CI can certify the LPA online.

Step 4: The CI submits your application to the Office of the Public Guardian (OPG). You’ll be notified once your LPA is registered.

Who can be my Certificate Issuer (CI) to certify my LPA?
The LPA form will have to be witnessed and certified by an LPA Certificate Issuer.
Any one of the following professionals in Singapore can be the CI for your LPA:- A medical practitioner accredited by the Public Guardian,
- A practicing lawyer (i.e. a Singapore solicitor qualified to practice Singapore law in a Singapore law firm), or
- A registered psychiatrist.
Note: Both of you and the CI will need to meet face to face and digitally sign your LPA using the Singpass application.

What if I need to appoint more than 2 Donees and/or 1 replacement Donee or have specific powers drafted for my LPA Form 1?
You will need to use LPA Form 2, which requires customised legal drafting. Usually, an LPA Form 2 set-up fee starts from $1,500.
